Requirements:
For minors under 18: 50 total hours of driving practice (including 10 nighttime hours) are required before applying for a Class E driver's license.
Must be signed by a parent or supervising adult (for minors).

Proof of Address

Certification of Address (Form HSMV 71120)
Requirements:
All applicants (minors and adults) need to provide two documents proving their Florida residency.
Accepted examples include utility bills, bank statements, lease agreements.
This form is used only if you cannot provide two proof of address documents and need a Florida resident to certify your address.
